Nurse Practitioner / Physician Assistant – Primary Care
Location: New York, NY
Schedule: Monday–Friday | No Nights or Weekends
Salary: $150,000 per year + Comprehensive Benefits
Setting: Federally Qualified Health Center (FQHC)

About Opportunity:
Our client is a Federally Qualified Health Center providing comprehensive primary and specialty healthcare services. This is an excellent opportunity for an experienced Nurse Practitioner or Physician Assistant with a strong primary care background who enjoys providing care to patients across lifespan.
Responsibilities:
- Provide primary care to pediatric, adolescent, and adult patients.
- Conduct patient histories, physical examinations, well-care visits, and gynecological evaluations as appropriate.
- Assess patients, establish diagnoses, and develop individualized treatment plans.
- Order and interpret diagnostic tests and prescribe medications as appropriate.
- Provide preventive care, health education, and chronic disease management.
- Coordinate referrals and ensure appropriate follow-up care.
- Collaborate with physicians, specialists, and other members of the healthcare team.
- Provide patient advocacy and participate in patient outreach activities.
- Maintain accurate and timely medical records and documentation.
- Participate in case conferences, peer review, and quality improvement initiatives.
- Provide guidance and support to medical assistants when serving at a satellite location.
- Follow HIPAA, facility policies, and applicable healthcare regulations.
Requirements:
- Graduate of an accredited Nurse Practitioner or Physician Assistant program.
- Master’s degree in Nursing or Physician Assistant Studies.
- Active, unrestricted New York State NP or PA license.
- Board Certified.
- Active DEA registration.
- Minimum of 2 years of clinical experience preferred.
- Experience providing care to pediatric, adolescent, and adult populations.
- Strong clinical, communication, and computer skills.
