Are you an RN looking to move beyond traditional bedside nursing while continuing to make a meaningful impact on patient outcomes?

We are seeking a Patient Care Coordinator – RN to provide proactive, patient-centered care to a high-risk patient population. This role works closely with providers to coordinate care, develop individualized care plans, educate patients, and help address barriers to treatment.

Job Responsibilities:

• Complete pre-visit planning and review daily patient appointments.
• Conduct proactive outreach to high-risk patients.
• Coordinate care and develop individualized care plans with providers.
• Assist patients with treatment goals and identify barriers to care.
• Educate patients on their care plans and available resources.
• Accurately document patient interactions and care plans in the EMR.
• Participate in Quality Improvement initiatives.
• Collaborate with Benefits Counselors, Outreach staff, and other healthcare team members.
• Perform other duties and special projects as needed.

Qualifications:

• Valid New York State RN License required.
• Prior clinical experience preferred.
• Care coordination or administrative experience preferred.
• Strong communication, organizational, and interpersonal skills.
• Ability to work effectively with providers and multidisciplinary teams.
• EMR experience preferred.
